I have also encountered this problem. The Sacrifice quest only gave Excalibur Umbra, but not Skiajati. However, the quest prompts that you need to equip Skiajati to proceed to the next quest. I sent a Skiajati in the cheat console to continue the quest.
And the Whispers in the Walls quest, I didn't see the reward email after completing it. I was confused why I didn't unlock Tennokai and Exilus slots after completing the quest. Later, I checked the wiki and found out that the reward email for Fibonnaci was missing. Melee Upgrade Segment and Mentor's Legacy MOD were also sent by myself in the cheat console
